Sugaring Consent Form

A sugaring consent form screens for the products and treatments that make skin fragile, explains normal redness and ingrown hairs, and records the client's signature before face or body sugaring.

No credit card required.

Gentler than wax, but the same screening

Sugar paste sticks to hair more than to skin and is used at body temperature, which is why many clients find it gentler than wax. Skin thinned by retinoids, isotretinoin or a recent peel can still lift, so the same questions apply.

Clients often switch to sugaring because they reacted to wax. The intake records that history, and the signed consent shows what they disclosed.

What to screen before sugaring

  • Topical retinoids or prescription acne products on the area
  • Isotretinoin in the past 6 months
  • Recent chemical peel, microdermabrasion or laser on the area
  • Sunburn, broken skin or active breakouts in the area
  • Previous reactions to waxing or sugaring
  • Diabetes or circulation problems (body sugaring)
  • Hair length: sugaring can grip hair from about 1/8 inch

Reactions to explain before treatment

  • Redness and small bumps for a few hours
  • Ingrown hairs in the following weeks
  • Rarely: skin lifting or bruising

Aftercare the client acknowledges

  • No hot baths, saunas or heavy exercise for a day
  • No tanning or exfoliation for a day or two
  • Loose clothing after body sugaring
  • Gentle exfoliation afterwards to prevent ingrown hairs

How it works

  1. Attach the form to your sugaring service.
  2. When a client books, they get the form by email and complete it on their phone.
  3. You see their answers and signature before the appointment, and they stay in the client record.

FAQ

Can clients on retinoids get sugaring instead of waxing?

Sugaring is gentler, but skin treated with retinoids can still lift. Most protocols avoid the area while retinoids are in use. The intake flags it so you can decide before the appointment.

Do I need a separate form if I offer both waxing and sugaring?

Not necessarily, but separate forms keep the service, risks and aftercare accurate for each. GlowForms attaches the right form to each service automatically.
