Spray Tan Consent Form
A spray tan consent form screens for allergies and breathing conditions, confirms the client knows the tan is not sun protection, and records their signature before the session.
No credit card required.
Why spray tans need consent
The active ingredient in most tanning solutions is DHA (dihydroxyacetone). The FDA lists DHA for external use on the skin but not around the eyes, lips or mucous membranes, or for inhalation. That is why clients should be offered eye protection, nose filters and lip balm, and why the form records that they were.
A spray tan is not sunscreen. The form records that the client understands they can still burn after their session.
What to screen before a spray tan
- Allergies to DHA, fragrance, nuts or other solution ingredients
- Asthma or other breathing conditions
- Pregnancy
- Eczema, psoriasis, broken skin or rashes
- Waxing or shaving in the past 24 hours
- A recent peel or exfoliation treatment
Reactions to explain before treatment
- Uneven or streaky colour, especially on hands, feet and dry areas
- Colour transferring to clothes or sheets in the first hours
- Rarely: itching or a rash from an ingredient
Aftercare the client acknowledges
- Wait to shower as long as your solution requires
- Wear loose, dark clothing straight after
- Avoid sweating, swimming and long baths for the first day
- Pat dry and moisturize daily so the tan fades evenly
- Use sunscreen: the tan does not protect against sunburn

- You see their answers and signature before the appointment, and they stay in the client record.
FAQ
Can pregnant clients get a spray tan?
Many salons ask pregnant clients to check with their doctor first. The form records the disclosure so you can decide.
Do I need a consent form for a mobile spray tan?
Yes. The risks are the same at a client's home or at a bridal party. GlowForms sends the form to each client's phone, so a group can sign before you arrive.
