Laser Hair Removal Consent Form
A laser hair removal consent form records the client's skin type, sun exposure, medications and treatment history, explains the risks and the need for several sessions, and captures their signature before the first pulse.
No credit card required.
Skin type and sun exposure change the risk
Lasers target pigment in the hair. When the surrounding skin also has a lot of pigment, from a deeper skin tone or a recent tan, the risk of burns and pigment changes goes up. Screening for that is how you choose the device and settings.
Who may operate a hair-removal laser differs by state: some allow trained estheticians, some require medical supervision, and some limit it to licensed medical professionals. Your consent form should match the rules where you practise.
What to screen before laser hair removal
- Fitzpatrick skin type (the Fitzpatrick versions of this template include it)
- Sun exposure, tanning or self-tanner on the area in the past few weeks
- Photosensitizing medications, such as some antibiotics
- Isotretinoin in the past 6 months
- Waxing, plucking or threading on the area recently (shaving is fine)
- Tattoos or permanent makeup in the treatment area
- Cold sores or herpes in the area, keloids, pregnancy
- Epilepsy or seizures triggered by light
Reactions to explain before treatment
- Redness and bumps around the follicles for a few hours to a couple of days
- Less common: blistering, burns, lighter or darker patches of skin
- Rarely: increased hair growth near the treated area
- Eye injury if protective eyewear isn't used
Aftercare the client acknowledges
- Avoid the sun and use SPF on treated areas
- No hot showers, saunas or heavy exercise for a day or two
- Shave between sessions; no waxing or plucking
- Keep to the session schedule you recommend; several sessions are needed
- Contact you if blistering or pain develops

- When a client books, they get the form by email and complete it on their phone.
- You see their answers and signature before the appointment, and they stay in the client record.
FAQ
Can estheticians perform laser hair removal?
It depends on the state. Some allow estheticians with laser training or certification, some require a supervising physician, and some limit lasers to medical professionals. Check your state board before offering it.
Why does the form ask about waxing but not shaving?
The laser needs hair in the follicle to target. Waxing and plucking remove it from the root, while shaving leaves it in place.
Does the client sign before every session?
Sign the full consent once, then update health, medications and sun exposure before each session. GlowForms can send the form again when the next session is booked.
