Microblading Consent Form

A microblading consent form records the client's medical history, the healing process and results you explained, and their signature before pigment goes into the skin. GlowForms sends it when the client books, along with the aftercare sheet afterwards.

No credit card required.

Microblading is a tattoo procedure

Microblading deposits pigment into the skin with a hand tool, so most states and local health departments regulate it as tattooing or body art. Many require a specific license, bloodborne-pathogen training and written client consent. Check your state and county rules.

Results are semi-permanent and vary from client to client. A signed form shows the client understood that healed strokes look softer than on day one, that a touch-up is usually needed, and that results can't be guaranteed.

What to screen before microblading

  • Blood thinners, aspirin or fish oil, and alcohol in the day or two before
  • Pregnancy or breastfeeding
  • Isotretinoin in the past year (many artists wait 6 to 12 months)
  • Retinoids, AHAs or peels on the brow area recently
  • Botox or other injectables near the brows in the past two weeks
  • Keloids or raised scarring
  • Diabetes, autoimmune conditions, chemotherapy or other conditions that affect healing
  • Skin conditions in the brow area: eczema, psoriasis, rosacea, very oily skin
  • Previous brow tattoo or microblading, and who did it

Reactions to explain before treatment

  • Redness, tenderness and slight swelling for a day or two
  • Brows look darker at first, then flake and can look patchy while they heal
  • Some strokes may not hold and need filling in at the touch-up
  • Less common: infection, an allergic reaction to pigment, or colour that shifts warmer or cooler over the years

Aftercare the client acknowledges

  • Follow the dry or moist healing method you give them
  • No makeup on the brows while they heal
  • No picking or scratching the flakes
  • No swimming, saunas or heavy sweating during healing
  • Sun protection on the brows, and no retinoids or exfoliants on the area

How it works

  1. Attach the form to your microblading service.
  2. When a client books, they get the form by email and complete it on their phone.
  3. You see their answers and signature before the appointment, and they stay in the client record.

FAQ

Can estheticians do microblading?

It depends on the state. Many treat microblading as tattooing and require a body-art or tattoo license and bloodborne-pathogen training, whether or not you hold an esthetics license. Check with your state board and local health department.

Does the client need to sign again for the touch-up?

Ask them to confirm their health answers again, since medications or pregnancy can change in the weeks between sessions. GlowForms can send the form again when the touch-up is booked.

Should microblading clients get before-and-after photos?

Photos protect you if the client later disputes the result. Use a version of the template with a photo release if you also want to post them.
