Eyelash Extension Consent Form

An eyelash extension consent form records the client's allergies and eye health, explains the adhesive and the risks of a reaction, and captures their signature before a full set or fill. GlowForms sends it to the client's phone when they book.

No credit card required.

Why lash artists need a signed consent

Lash adhesive is cyanoacrylate-based and is applied millimetres from the eye for an hour or more. Most clients have no problem, but sensitivity can develop after many uneventful sets, so the questions need asking every time, not only at the first visit.

Swelling or itching usually appears hours later, at home. A signed form shows what the client disclosed, that the risk was explained, and what aftercare they agreed to.

What to screen before lash extensions

  • Previous reactions to lash adhesive, cyanoacrylate glues, latex or medical tape (used for under-eye pads)
  • Eye conditions: conjunctivitis, blepharitis, styes, dry eye or watery eyes
  • Eye surgery in the past few months, such as LASIK or cataract surgery
  • Contact lenses (removed before the appointment)
  • Conditions that affect natural lashes: alopecia, trichotillomania, chemotherapy
  • A recent lash lift or tint
  • Whether the client can lie still with eyes closed for the length of the service

Reactions to explain before treatment

  • Mild stinging or watering from adhesive fumes during application
  • Red patches on the whites of the eyes after the appointment, usually from fumes or pad pressure
  • Allergic reaction: itchy, puffy or red eyelids, sometimes appearing a day or more later
  • Damage to natural lashes if extensions are too heavy, or if the client picks or pulls them

Aftercare the client acknowledges

  • Keep lashes dry for the time your adhesive requires
  • Oil-free cleanser and makeup remover around the eyes
  • No mascara on the extensions, and no lash curlers
  • Brush lashes gently every day; never pick or pull them
  • Contact you, and a doctor if needed, if itching, swelling or pain develops

How it works

  1. Attach the form to your eyelash extensions service.
  2. When a client books, they get the form by email and complete it on their phone.
  3. You see their answers and signature before the appointment, and they stay in the client record.

FAQ

Can a client become allergic to lash glue after years without problems?

Yes. Sensitivity to cyanoacrylate can develop over time, which is why the allergy questions are asked before every set. A client who reports new itching or swelling should be seen by a doctor before their next fill.

Do I need a new consent form for every fill?

A full consent once, plus a quick health update at each visit, is common practice. GlowForms sends the form automatically when the client books, so updated answers are on file before every appointment.

Should I patch-test new lash clients?

Some artists patch-test with a few lashes before a full set. Follow your adhesive manufacturer and insurer, and record what you did in the client record.
