Microneedling Consent Form

A microneedling consent form screens for conditions that make needling unsafe, explains redness, pinpoint bleeding and infection risk, and records the client's signature before the session.

No credit card required.

Check scope of practice first

Microneedling breaks the skin. In some states it is outside an esthetician's scope of practice or limited to certain needle depths and devices. Check your state board before offering it — no form makes an out-of-scope service safe.

Where you can offer it, a dedicated form documents screening for infection and scarring risks that a facial form doesn't cover.

What to screen before microneedling

  • Active skin infection, open wounds, active acne or cold sores in the area
  • History of keloid or raised scarring
  • Blood thinners or bleeding disorders
  • Isotretinoin in the past 6 months
  • Pregnancy or breastfeeding
  • Autoimmune conditions or diabetes that slow healing
  • Recent sunburn, peels, laser or injectables in the area

Reactions to explain before treatment

  • Redness similar to a sunburn for one to three days
  • Pinpoint bleeding during treatment
  • Mild swelling and dryness
  • Less common: infection, pigment changes, cold-sore flare

Aftercare the client acknowledges

  • Keep the skin clean and avoid touching it
  • No makeup for the time you advise
  • No sweating, swimming or saunas for a day or two
  • SPF daily and no active products until healed

How it works

  1. Attach the form to your microneedling service.
  2. When a client books, they get the form by email and complete it on their phone.
  3. You see their answers and signature before the appointment, and they stay in the client record.

FAQ

Can estheticians do microneedling?

It depends on the state. Some allow it within limits, others reserve it for medical providers. Check your state board's current rules before offering it.

How long before microneedling should a client stop isotretinoin?

Many protocols wait at least 6 months. Follow the device manufacturer's guidance and the client's prescriber.

Should microneedling clients sign a photo release?

Only if you plan to use their photos. GlowForms has versions of this template with a separate photo and video release.
